DOSSIER - Citocromo P450: un panorama desde la clínica
Resumen
El citocromo P450 es un sistema enzimático que participa en el metabolismo oxidativo de numerosos grupos de fármacos. La mayor parte de las interacciones farmacocinéticas de importancia clínica se vinculan con alteraciones en el metabolismo, en el que el citocromo P450 juega un papel central. En este artículo se describe la estructura y funcionamiento de este sistema enzimático, y los mecanismos involucrados en las interacciones metabólicas. Se plantean, además, dos importantes aspectos vinculados con este complejo sistema, como son el polimorfismo genético que exhibe y la influencia que sobre su funcionamiento pueden tener los factores ambientales tales como los alimentos y algunos contaminantes. Se propone, finalmente, una reflexión sobre la importancia del citocromo P450 desde el punto de vista de la práctica clínica.

THE CYTOCHROME P450 AS VIEWED FROM CLINICAL PRACTICE
Summary
The cytochrome P450 is an enzymatic system involved in the oxidative metabolism of a large number of drugs. Most clinically significant pharmacokinetic interactions are related to alterations in drug metabolism, in wich the cytochrome P450 is implicated. The present article describes the structure and functioning of this enzimatic system and the mechanisms involved in metabolic interactions. Two important aspects related to this complex system are underlined: the genetic polimorphism and the influence of the eviroment in the activity of the enzymes. Finally, the importance of the P450 system is analized from the point of view of clinical practice.
